FALLSBURG — Details were few this week, but an incident at Sullivan Correctional Facility in Fallsburg Monday ended with the death of an inmate and injuries to three corrections officers.

NYS Dept. of Correctional Services spokesperson Linda Foglia said a man in his 50s passed away Monday morning at the maximum-security state prison.

“An investigation is under way into the inmate's death,” she said, led by the NY State Police, with the department's cooperation.

On Tuesday, she released the name: Karl Taylor, 52, who was serving a term of 27 ½ to 55 years for sex crimes (Rensselaer County) and aggravated harassment of an employee (Chemung County).

In 1995, Taylor was convicted and sentenced in Rensselaer County for three counts of rape in the first degree and one count of sexual abuse in the first degree. In 2002, he was convicted and sentenced in Chemung County for one count of aggravated harassment of an employee while incarcerated at the Southport Correctional Facility.

Taylor entered the state prison system on March 24, 1995 and was transferred to Sullivan Correctional Facility on November 25, 2014. He was eligible for parole consideration in 2020.

Sources say there was an altercation started by Taylor on Monday, but authorities would not confirm or deny such.

However, the spokesperson for the union, James Miller of the NYS Correctional Officers PBA, confirmed that three corrections officers were sent to an urgent care facility as a result of the incident.

One had minor hand injuries and was released. Two, though, were transferred to Catskill Regional Medical Center in Harris, he said. One suffered two broken bones, and both had concussions. However, they too were released later that day.
